Synopsis

This presentation examines the challenges faced by young people transitioning from out-of-home care (OOHC) in Australia, where they often experience significant disadvantages. Semi-structured interviews with 14 staff from a Victorian care organisation identified barriers to effective transition planning, including difficulties with accommodation, reluctance from young people to engage, and late planning. Staff recommended improvements such as creating specialist transition planning roles and referenced best-practice programs. The findings provide key insights to guide future research, policy, and practices for better transition pathways for young people leaving OOHC.
