There are new data collection and reporting requirements for Sixth Community Pharmacy Agreement (6CPA) funded Dose Administration Aid (DAA), Staged Supply, MedsCheck and Diabetes MedsCheck Pharmacists providing these services will need to record patient data at the time of the initial service and at a six-month follow up consultation to demonstrate the important impact of these services on patient health outcomes. Delivered by Chris Campbell, this session aims to equip pharmacists providing 6CPA programs with the necessary skills and knowledge to maximise their delivery.
Learning Objectives:
• Identify new 6CPA Staged Supply program requirements for Stage Supply, DAA, MedsCheck and Diabetes MedsCheck
• Describe how to conduct each 6CPA program in line with the revised program requirements
• Discuss the rationale for health outcome data collection
• Identify patients who are eligible for 6CPA-funded programs based on the revised patient eligibility criteria
• Articulate the linkages between 6CPA programs
• Complete staged Supply services in accordance with 6CPA program rules.
